Form Troubleshooting: Submission Not Appearing in Lead Center
Use this checklist to diagnose why a form submission is not appearing in Lead Center and quickly restore reliable lead capture.
Symptoms
A user submits a form, sees a success state, but the record does not appear in Lead Center.
Quick Diagnostic Checklist
- If the submission is from a non–DigitalPilot form on your site, confirm Capture lead emails into DigitalPilot is on in Settings → Tag options (admins only). Skip this for DigitalPilot Form Builder forms.
- For DigitalPilot Form Builder, confirm the form is the latest saved version in the editor.
- Confirm the DigitalPilot tracking tag is installed on the page.
- Submit a fresh test using a unique business email.
- Search Lead Center with date range and exact email value.
Common Root Causes
- Tag capture off (third-party forms only): For leads from your own non–DigitalPilot site forms, turn on Capture lead emails into DigitalPilot in Tag options. Not applicable to DigitalPilot Form Builder submissions.
- Wrong workspace/tag: The form is connected to a different workspace.
- Blocked domain rule: Email was filtered by personal-domain blocking.
- Caching/deployment mismatch: Old embed code is still live.
- Ad/script blockers: Browser extensions can prevent tracking calls during tests.
How to Validate the Fix
- Run a new incognito test after each change.
- Confirm submission appears in Lead Center within your test window.
- If using notifications, confirm Slack/webhook events also fire.
Escalation Data to Collect
- Form URL and workspace/tag name.
- Test email and test timestamp.
- Screenshot of form success state.
- Whether browser extensions were enabled during testing.
